I, __________________________ (print name), hereby acknowledge and agree to the following terms before participating in a test ride of an electric bicycle ("e-bike") provided by OHM Electric Bikes:

1. ASSUMPTION OF RISK: I understand that riding an e-bike involves inherent risks that may result in injury, death, or property damage. These risks include but are not limited to: falling, collisions, mechanical failures, road conditions, and my own physical condition and skills.
2. HELMET REQUIREMENT: I agree to wear a helmet at all times during the test ride.
3. COMPLIANCE WITH LAWS: I will follow all applicable traffic laws, rules, and regulations while riding the e-bike.
4. PROPER USE: I will use the e-bike in a responsible manner and will not attempt stunts, tricks, or riding beyond my skill level.
5. NO IMPAIRMENT: I confirm I am not under the influence of alcohol, drugs, or any substances that could impair my ability to safely operate the e-bike.
6. WAIVER AND RELEASE: To the fullest extent permitted by law, I RELEASE, WAIVE, AND FOREVER DISCHARGE OHM Electric Bikes, its owners, employees, representatives, and agents from any and all claims, liabilities, causes of action, damages, losses, costs, or expenses arising from or related to my participation in the test ride, regardless of cause.
7. INDEMNIFICATION: I agree to indemnify and hold harmless OHM Electric Bikes from any liability, claims, or damages related to my use of the e-bike.
8. PROPERTY DAMAGE: I accept responsibility for any damage I cause to the e-bike during the test ride, beyond normal wear and tear.
9. MEDICAL TREATMENT: I authorize OHM Electric Bikes to seek emergency medical treatment on my behalf if necessary, and I accept financial responsibility for such treatment.

I HAVE READ THIS RELEASE AND WAIVER OF LIABILITY AGREEMENT, FULLY UNDERSTAND ITS TERMS, AND SIGN IT FREELY AND VOLUNTARILY.
